Every three to five years, local health boards lead a discussion regarding public health issues in the community. This discussion, done in partnership with health care providers and community partners, seeks to involve residents, business owners, and community stakeholders.
This process results in the Community Health Needs Assessment and Health Improvement Plan (CHNA/HIP) report. The process and report are required by the Iowa Department of Public Health to:
- receive funding from the State
- to fulfill the duties of the Dubuque County Board of Health
