Policy
Sunnycrest is owned and operated by the County of Dubuque. Priority for admissions to Sunnycrest Manor is given to persons who are of legal settlement in Dubuque County and who are financially indigent. Eligible veterans who meet the criteria for VA contract services are also eligible for admissions irregardless of legal settlement. Applicants with special needs that cannot be met in traditional long term care settings and have a history of residency in Dubuque County may be also be considered for admission. Admissions for all eligible persons is based on room availability and the facility’s ability to meet the applicant’s needs.
County MI/MR Policy (Institutional Accounts)
Applicants must meet the Federal/State criteria for the level of care for which they are applying and for which Sunnycrest holds licensure (RCF, NF, Skilled, ICF-MR). No person under the age of sixteen will be admitted to Sunnycrest without prior written approval from the Iowa Department of Inspections and Appeals. In accordance with Iowa Department of Public Health code 1.4(2), Sunnycrest Manor will not accept persons who are in an active state of alcoholism, drug addiction, mental illness or who have medical conditions which require specialized care or treatment. Sunnycrest Manor will not accept persons who are unduly disturbing or dangerous to themselves or to others. 9IDPH 1.4 (2)
This facility prohibits discrimination against current or prospective residents on the basis of race, color, sex, religion, handicap, veteran status or any other legally protected characteristic. It is the policy of the facility to admit and promote health regardless of the foregoing characteristics. The facility is firmly committed to a policy of equal opportunity in all aspects of resident and family relations, including care, individual requests, social expectations and interactions.

Procedure
Referrals to Sunnycrest are accepted by the facility social workers, Administrator or other designated management personnel.
The Application for Admission, releases of information and Dubuque County Application for Assistance are provided to the referring party, the person interested in admission or their responsible party. A tour of the facility is encouraged for those who are unfamiliar with Sunnycrest.
When a person (“applicant”) conveys an interest in placement at Sunnycrest pertinent information will be gathered about the applicant including necessary medical and mental health information. The Sunnycrest Application for Admission and the Dubuque County Application for Assistance will be reviewed (contains county legal settlement, residency and financial information). Further information to determine legal settlement may be obtained through the County CPC Administrator. Information on Medicaid eligibility may be obtained through the Department of Human Services.

One or more representatives from the facility Admission Committee will conduct a pre-placement interview with the applicant and responsible party to ascertain the applicant’s medical and psychosocial needs, lifestyle, and preferences in order to determine Sunnycrest’s ability to meet the applicant needs with existing services, utilization of community services or the development of adequate programs.
The facility Admissions committee consists of representatives from nursing, social services, dietary, activities, and the facility Administrator. The Admissions committee will review the gathered applicant information and information from pre-placement interview and has the authority to approve or deny admissions to the facility. Priority status is given to applicants who have legal settlement in Dubuque County who meet Medicaid eligibility; and to eligible Veterans who qualify for VA contract services. All other applicants must be approved by a minimum of two members of the Dubuque County Board of Supervisors.
Acceptance into Sunnycrest Manor does not impact eligibility for other Dubuque County funding.
The Admission Committee decision and the reason for decision will be relayed to the applicant/family/referring party by facility social services or designated management personnel.
Prior to admission applicants accepted for admission must have physician orders for the level of care needed, medication, treatment and therapy services. A TB test must be administered within 6 months before date of entry to the facility.
Applications and information gathered through the screening process for applicants accepted for admission will be retained in the resident’s legal and medical file. Applications and information for applicants denied admission will be retained in the Admission Committee files located in the social work office.
